Should you be interested, kindly fill in the application form (please print and fill) and walk-in to one of our offices or authorized branches with the copies of the following:

• Passport photocopy
• 12 passport-size photographs (10 for school, 2 for visa)
• Birth Certificate
• Medical Health Certificate and chest x-ray 
• Education certificates/transcript of records
• Criminal clearance report 
• Affidavit of support of the person supporting the applicant’s study 
• Financial Statement From Bank regarding capabilities of the person funding the study of the applicant 
• Certificate of good moral character (1 from either Principal/professor or Dean)

(Items 4-8 need to be authenticated by the Embassy of the country you are going to)
